Office Hours:
Our physicians see patients by appointment only. The offices are open Monday through Friday from 8:00 a.m. to 5:00 p.m. with each physician offering office hours on different days of the week.
We understand that your schedule may change and you may need to cancel an appointment. Please inform us as far in advance as possible so we are able to make that time available for the care of other patients.We also ask for your understanding when emergencies with other patients arise, requiring us to delay or reschedule your appointment.
X-Ray Services:
We have on-site x-ray services available to assist in your care. If you have had recent x-rays for this orthopaedic problem please bring them with you. If these were taken at Munson Medical Center, you must call the Radiology Department at 935-6400 and request your x-rays 24 hours in advance.
Prescription Refills:
Please call the office between 8:00 a.m. and 3:00 p.m. to request a prescription refill before you run out of medicine. Please do not call after 3:00 p.m. or on weekends because our "on call" physician does not have immediate access to your medical records and will not be able to prescribe a refill. No refills will be given if you have not seen the doctor within six months.
Disability Forms:
There is a $15.00 charge for completing each form.
Fees and Insurance:
We require payment in full on the day of your visit. For your convenience, we accept cash, personal checks, Visa and Mastercard. Payment programs should be arranged prior to your date of surgery.We participate with BCBSM, BCN and Medicare. We do require HMO/PPO patients to be referred from your primary physician and that a referral form be provided at the time of your appointment. You should be prepared to pay any co-pays at the time of your visit.In order to file your claim for worker's compensation, auto accident or personal liability, we must have written acknowledgement from the responsible party in advance for treatment and payment. Workers' compensation patients must include both the employer's and the insurance company's names and addresses. Failure to bring a letter will result in your appointment being rescheduled. If you elect to have surgery, you will be told the fee. It will be your responsibility to contact your insurance company and let them know of the surgery and the fee. You will be responsible for any balance not paid by your insurance. Minors must be accompanied by an adult.
Non-Covered Services:
Some of the services provided by Great Lakes Orthopaedic Center may not be covered, either partially or entirely, by some health insurance companies. In that case, you will be billed by our office for those services. We suggest that you contact your insurance company to verify coverage for specific services.
Telephone Messages:
Our staff is trained to assist you with your concerns or questions. Should these require a physician's attention, a message will be taken and a physician will return your call at his earliest opportunity. If there is an emergency, a physician will be contacted immediately.
Emergencies:
Our physicians rotate "on call" coverage after hours and on weekends. If you have a medical concern after hours, our "on call" physician can be reached by calling Munson Medical Center's switchboard at 935-5000.
If you have a serious or life-threatening emergency, go directly to the nearest emergency room or call 911.
